Please inform the receptionist of any changes to your address, phone number, and/or insurance information. You are responsible for providing us with your correct billing information.

Copays are due at the time of check-in, unless prior arrangements have been made.

Please make an appointment to see the doctor. Due to a high volume of patients needing to see Dr. Trehan, he may not be available to see you if you do not have a scheduled appointment.

For all missed or cancelled appointments with less than 24 hours’ notice, you will be charged a $25 cancellation fee. Appointment reminders are a courtesy. Should you not receive a reminder call, it is still your responsibility to remember your appointment.

 Please give us 24 – 48 hours notice on all prescription refills.

You will be charged a $25 fee for each medication requiring prior authorization. 

Please note that this office charges a $25 fee to fill-out any, and all paperwork on your behalf, to cover our time and cost.

Please note that if you decide not to be physically seen by the provider in the office, our office charges a Telehealth visit fee to treat patients over the phone. 

A $25 charge will be added to your account for all returned checks.
